Steady Regress: Missing stocks at Arua R.R. Hospital, again!

Again, a hospital under the ruler of President Yoweri Kaguta Museveni gets sinking with either the staff getting sacked, missing equipment or allocated funds dispersing. Arua Regional Referral Hospital we’re founded in 1939. So this is not built on the merits of National Resistance Movement, this is a hospital that we’re running before NRM existed.

Much funding in recent years for the day-to-day operation has been through the SUSTAIN and Anti-HIV/AIDS program of USAID. That lasted for 5 years and we’re run by partners University Research Co. LLC and Integrated Community Based Initiatives (ICBI). This lasted until June 2015.

Arua Referal Hospital

The missing equipment now at hospital isn’t new as in February 2016 this we’re reported:

“Arua Regional Referral Hospital is struggling with an acute drug stock out. Information obtained by URN shows that the hospital has been operating without essential drugs for the last two months” (…) “As a result, patients are diagnosed and referred to private facilities to buy the necessary medicine. Christine Atizuyo, an expectant mother told URN that she was asked to pay Shillings 10, 000 and buy medicine from a private facility, when she visited the hospital for antenatal care recently” (…) “Dr. Darlington Akusa, a cancer specialist at Arua Regional Referral Hospital, says they are struggling to provide health services to patients due to lack of drugs. He says currently they only examine patients and prescribe for them medication, which they have to buy from private facilities” (Ariaka, 2016)

So that the regional hospital lacking stuff isn’t new, it is just systematic the way the NRM sees fit for the importance of the Health Sector and the well-being of its citizens. Surely the NRM might have funds for vaccines and medicines through GAVI, but they have stolen that in the past and surely in the present if needed. So with the knowledge of this the government continues to pass the buck and give enough to supply the hospital.

This will happen again as long as the NRM regime continues to rule. They just don’t care instead they uses fortunes on expenses and luxurious lavish lifestyle for the current Members of Parliament. Peace.

Press Release: Uganda’s Routine Immunization ranked among the lowest in the East Africa (14.03.2016)

KAMPALA, March 14: “In God we trust, all others bring data,” a quote attributed to American professor and statistician William Edwards Deming, is widely used to underscore the role accurate data play in building credibility.

As a result of the lack of verified, up-to-date data, a report released ahead of the ministerial conference on immunisation in Africa last month ranked Uganda’s routine immunisation coverage as among the lowest in the East African sub-region.

The report which tracks immunisation progress and challenges across the continent since the Global Vaccine Action Plan (GVAP) was launched and entitled, “Fulfilling a promise: Ensuring immunisation for all in Africa”, rates Uganda’s 2014 diptheria-tetanus-pertussis (DTP3) coverage at 78 per cent compared with 80 per cent for the Democratic Republic of Congo (DRC), 81 per cent Kenya, 97 per cent for Tanzania and 99 per cent for Rwanda.

Coverage with the third dose of DTP is used to measure the strength and reach of a country’s routine immunisation programme.

The DTP3 target for countries was 90 per cent for national coverage by 2015 or at least 80 per cent coverage in every district. Experts recommend that children receive DTP three times at appropriate intervals.

Even when the Ugandan Gov. have the funds for health care and medicine; they still mismanage the funds or does not procure the necessary kits, staff or proof of use of the Global Funds; findings from the Audit Report of February of 2016 from The Global Funds!

There been reported how the Global Fund has gone through and report how the donor funds to Government of Uganda (GoU) and the Ministry of Health. As the Value for Money way of auditing and describing the state of the programs that has been funded by this donor funding; this is especially against the diseases like Tuberculosis, HIV/AIDS and Malaria. This is the situation of the funds and how it was used. This is interesting to see how the Government of Uganda has been coming to accountability and responsibility toward the procurement and accessing the monies, as they was not using the allocated funds or unaccounted for. There is questions for why certain projects are so slow in procurement and why there is too little of specialized kits in the National Medicine Stores (NMS) as they had budget for a dozen more than; when the audit was happening. That is a worrying sign. But look at the quotes from the Global Fund report from February 2016, and see what the important pieces from it are!

Aids Support

“The Global Fund support in Uganda:

Since its inception in Uganda in 2002, the Global Fund has signed a total of 20 grants amounting to USD 1 billion, USD 623 million of which had been disbursed to the country at the time of the audit” (…)”The grants are implemented by two Principal Recipients, The Ministry of Finance Planning and Economic Development and The AIDS Support Organization (TASO). The Ministry of Finance has delegated responsibilities with respect to implementation of the grants to the Ministry of Health” (…)”Approximately 90% of Global Fund grants to Uganda are spent on the procurement of medicines and health products. The Secretariat’s Pooled Procurement Mechanism procures all health commodities with the exception of tuberculosis drugs which are procured by the Global Drug Facility” (P: 4, 2016).

Ratings of the work:

The rating of the operations tells a story on how the services are delivered in the country. As the Programmatic and Performance where you can see the accuracy and support decision making to check the quality service, second part is the Financial and Fiduciary how to use the actual grants and check them in an effective manner, this two both the Programmatic and Performance, and the Financial and Fiduciary is handled in a Partial Plan to become Effective (P: 5, 2016).

The Health service and Products which is the ability of the supply chain, deliver services, account the quality assured medicines and health manners in timely manner; the second rated work is the Governance, Oversight and Management it is the quadrate and the effectiveness of the grants and implementations of the arrangements. These two parts is not run effective by the government (P: 5, 2016).

Kitgum NMS

Executive Summary:

“The Global Fund has signed a total of 20 grants amounting to USD 1 billion, USD 623 million of which has been disbursed to the Republic of Uganda since 2002” (…) ”Approximately 90% of grant funds are spent on the procurement, storage and distribution of health commodities. The Global Fund’s Pooled Procurement Mechanism buys the majority of the medicines and health products on behalf of the country, which has significantly improved procurement timelines and reduced commodity prices” (…)”Uganda has made progress in the control and treatment of HIV, tuberculosis and malaria with a reduction in new infections and/or incidence. However, if unaddressed, pervasive stock-outs of key medicines at all levels will result in treatment disruption for patients. Seventy per cent of the 50 health facilities visited during the audit reported stock-outs of at least one critical medicine, with HIV drugs being the most affected of the three diseases” (…) ”Differences of USD 21.4 million were noted between book and actual stocks at the National Medical Stores for 15 commodity types procured by the government and the Global Fund. The audit could not apportion the variance between the government and the Global Fund since the stores’ inventory system does not segregate physical stocks by source” (…) ”16.5 million condoms that should have been distributed for free were sold through social marketing. The funds generated from the sales (USD 0.2 million) remain unaccounted for” (P: 6, 2016).

Executive Summary Part II:

“The country’s change of HIV treatment policy and scale up plans have increased the number of patients eligible for treatment without a corresponding increase in government funding. This will result in a treatment funding gap of at least USD 90 million in 2016 if not addressed.

Consequently, the Global Fund is ‘front-loading’ commodities planned for 2016/17 to 2015 to address medicine shortages” (…)”Twelve per cent out of the 50 facilities visited were performing HIV tests with expired test kits and, contrary to national guidelines, 14% of facilities visited did not perform confirmatory tests on clients diagnosed as HIV positive. This raises the risk of clients getting false HIV results” (…)”The Secretariat, in collaboration with the Ministry of Health, has introduced data quality assessments. Vacant positions are to be filled to address the data related issues. However, funding for tools, training and supervision remains a challenge” (…)”There was also weak management of advances with some remaining outstanding for over 20 months. Value added taxes amounting to USD 0.3 million had also not been refunded to the programs. The audit identified expenses for which there was not adequate supporting documentation, amounting to USD 3.9 million” (…)”While the country lacks adequate funding to cover key activities, it has a low absorption of the limited grant funds that are sent to the country. The OIG noted that only 46% of funds disbursed to the Ministry of Finance between January 2013 and June 2015 had been spent at the time of the audit” (P: 7, 2016).

NMS Amuria

Pervasive stock-outs:

“70% of the health facilities reported stock-outs of anti-retroviral medicines and HIV test kits of between three weeks and four months” (…)”68% of facilities reported stock outs of anti-malaria medicines and test kits in the previous six-month period” (…)”64% of the facilities reported stock-outs of TB medicines of between one week and three months” (P: 9, 2016).

Use of medicines to treat other diseases: The audit noted that 32% of the 50 facilities visited treated 1,254 Hepatitis B patients with anti-retroviral medicines. The quantification of anti-retroviral medicines does not take into consideration their use for the treatment for Hepatitis B patients. This has contributed to stock-outs of anti-retroviral medicines for HIV patients who are the primary target of these medicines” (P: 9, 2016).

Gaps in HIV counselling and testing practices:

“Twelve per cent out of the 50 facilities visited were performing HIV tests with expired test kits” (…)”Contrary to national guidelines, 14% of the facilities visited did not perform confirmatory tests on clients diagnosed as HIV positive” (P: 11, 2016).

Inadequate and ineffective condom procurement and distribution processes:

“Condoms that should have been received in country in 2011 were only received in late 2013 due to a protracted procurement processes” (…)”Contrary to the grant agreement, 16.5 million condoms that should have been distributed to users for free were provided to Marie Stopes Uganda, a contractor, by the Ministry of Health and sold through a social marketing mechanism” (P: 12, 2016).

Abim Hospital 2014 P3

Subsidized anti-malarial medicines not accessible and affordable:

“Consequently, medicines are sold above the recommended price that is UGX 5,000 and not UGX 3,500” (…)”there is no instituted mechanism to ensure that the subsidized medicines are distributed outside the big cities to malaria endemic areas” (P: 12, 2016). “Key positions budgeted for under the Global Fund grants also remained vacant: for example, 17 out of the 43 pharmacists and HIV, TB and malaria focal points for the regional performance monitoring teams were not at post during the audit” (P: 13, 2016).

Difference between what they have funded to get and what they had at NMS:

Between Global Fund commodities issued by national Medical Stores and received by health facilities: The National Medical Stores inventory system indicated that 3.7 million test kits had been issued to a facility, but the facility recorded a receipt of only 3,000 kits. While the National Medical Stores indicated that the variance amounting to USD 2.41 million may be due to errors in the inventory management system, this could not be verified by the OIG auditors. The variance also affects the closing quantities based on the inventory management system and actual stock at the national medical stores” (P: 14, 2016).

“In-country quality assurance of medicines: The National Drug Authority charges 2% (amounting to USD 3.8 million from January 2013 to June 2015)39 of the “free on board” value of medicines and pharmaceutical products for in-country quality assurance. While bed nets and condoms had been tested, there was no evidence that medicines (including anti-malaria and anti-retroviral) supplied by the Global Fund were tested by the Authority” (P: 15, 2016).

Questionable value for money: Charges of USD 3.8 million (from January 2013 to June 2015) by the National Drug Authority for testing of medicines, for which there is no evidence that testing actually happened” (…)”Cancellation of an order for the purchase of HIV test kits under the Pooled Procurement Mechanism which has resulted in a loss of USD 427,500. The manufacturer has indicated that the commodities have already been manufactured and cannot be supplied to any other country due to level of customization requested by the Ministry of Health” (…)”Payments amounting to USD 254,921 related to value added taxes that has not been refunded by government” (…)”The implementers incurred ineligible payments amounting to USD 93,400. These related to payments for activities not included in the approved grant budget, or excess payments to service providers” (P: 19, 2016).

Mengo Hospital needs funds

Afterthought:

This here shows worrying signs as the Health Care and Global Funds is either not utilized or misused, understocked even when the NMS is supposed to have dozen of kits for instance. The amount of monies not allocated even when budget for. That is a normal issue for the Government of Uganda under the NRM-Regime.

Just like the condoms that was supposed to enter Uganda in 2011, arrived in 2013. That proves the ability of the government to stall the procurement even when they have donor-funding to get the necessities. One key issue is that health facilities are lacking the necessary medicines for the treatment of Tuberculosis, HIV/AIDS and Malaria. The planning for securing the allocations is also lacking, therefore the planning and allocations is missing even when the funding from Global Funds is there, showing that the transactions between the Government of Uganda, National Medical Stores(NMS) and the International Companies who make the kits and medicine. Another factor is the longstanding time the health facilities are without needed medications and that should be worrying for the Ministry of Health.

The worst thing about this is that people who need the treatment have to wait for it or not get it in time as the health care facilities do not have it. That is the thing that worries me while reading through it, seeing the person who needs the care and medicine not getting it. So the Government of Uganda, Ministry of Health and National Medical Store (NMS) has a decent job to do. Especially since the matter of making procurement procedure and facilitate together with the different parts of governments organizations to deliver to the patient at a facility or the pharmacy. What NRM’s #Steady Progress really is…

We all know that the election is going on and that NRM have a campaign slogan as such: “Steady Progress”. This is the time when NRM is telling in each district they visit about the 29 years of “steady progress”. Well, I will not, I am not NRM. And not a great fan of the NRM. As those of you who read my page/blog should understand by now. I will show another story of the #Steady Progress have been under the NRM and Yoweri Kaguta Museveni’s presidency.

#Steady Progress under the NRM-Regime is that there been violence and arresting the opposition leaders with different claims. And have rhetoric that attacks others then look into their own missteps. Especially when Mzee hear the reports of the violence were from NRM-Supporters together with the Police in an area (Ntungamo clashes for instance). There isn’t the first time the President Museveni spreads fear in the name of democracy and not the last. He can put his words into a leopard’s mouth and see if the animal bites.

#Steady Progress is to have close hospitals in the weekends and having no nurses at the referral hospital in the districts. #SteadyProgess is to suspend health workers after showing the terrible working condition of the Health Care facility.

Burundi Report Police

#Steady Progress is to use the colonial law of preventative arrest when it fits the NRM-Regime and if it fits the new law Public Order Management Act then the Police will take care off or silence Political Opposition leader, Human Rights Activist or even random pedestrians.

#Steady Progress is that the police tells the world and journalist on how to conduct themselves and how to be a part of campaign convoys of the opposition leaders.

#Steady Progress is to have terrible roads in and around the villages. #Steady Progress is to finally have rail system working between Kampala – Namanve, but not for the rest of the country, and it had to happen right before the election.

#Steady Progress is to land as hunting-grounds or for services rendered for the government either to the likes of Aga Khan or other foreigners, even trade of forests and mineral rich land, to get it pocketed while the Government of Uganda can.

#Steady Progress is to borrow more and more money to fund the aid deficit created by the “Anti-Gay-Bill” fiasco. That left a vacuum from foreign interest together with the strategic loss of partners to sustain their aid to the country and picking other feasible projects there.

#Steady Progress is give money to religious leaders to secure their congregation who their God has picked as leader and who their lord has anointed to the position.

#Steady Progress is to get a bigger government and more ministries so that all the ones closes and most loyal get a place in the central government.

#Steady Progress is to give a person the role of ministry without a portfolio or office

#Steady Progress is to use more on private plans and military equipment then on basic school kits, school buildings or even medical supplies.

#Steady Progress is for the President to claim again and again that he is the only Mzee who can keep the country safe and keep control of the army.

#Steady Progress is to have old medicine or copy medication instead of having extra supplies through the NMS.

#Steady Progress is to have ghost-workers, ghost-voters, ghost-schools, ghost-roads or ghost-expenditure.

#Steady progress is to fire all of the UNRA workers and getting the funding for bigger projects through the World Bank cancelled.

#Steady Progress is to see that Uganda Human Rights Commission loses it funding from it’s donors.

#Steady Progress is to hire “Crime Preventers” as securing votes and create havoc in other candidate’s rallies to make sure that people stay loyal to the Mzee, and also ship them from NRM rally to NRM rally to make the party look more popular.

#Steady Progress is to have more and more car accidents on the bad roads, even lose the lives of more Members of Parliament due to bad roads in the districts.

#Steady Progress is for the rich and for the wealthy to take their family members or themselves to health facilities abroad to heal and prosper.

#Steady Progress is to borrow money on the future-oil monies and trade of the oil refinery from Russian state-owned company to buy military equipment.

#Steady Progress is to lose battalions and soldiers in Somalia and leave new recruits with little or no supply. The army in Somalia has to sell equipment for food and necessities; instead of fighting the terrorist group Al-Shabab.

#Steady Progress is that landslides after heavy rains and floods have no covering or security funds from the government nor any restrictions or helping agents to save lives on that matter.

#Steady Progress is given funds of 51 billion to the post-election violence for the Police and army for the “excuse” to secure the safety and peace in the country.

#Steady Progress is to tear down other candidates posters while sealing off and securing with Police force the incumbent presidential candidates ones on the walls.

Ready to Move with Sevo Music Campaign for 2016 P5

#Steady Progress is to use a tiny fortune to make a gig-song and have singers on the campaign trail.

#Steady Progress is to pay boda-boda men in town and villages to not drive to opposition rallies.

#Steady Progress is to have a rising inflation and devalue the currency before and after every election time, leading the results for more expensive fuel, food and living prices in general. 

#Steady Progress is to send SMS’s to everybody to tell them to VOTE MZEE, even if they don’t are NRM members or belongs with the Party, instead he sends to all Ugandans in the companies to gain support.

#Steady Progress is to pay boda-boda drivers to drive people to Mzee’s venues, give away t-shirts, give away free fuel, feed the public food and give money to the attendance of the crowd.

We can all see and this a gist of the actions and events leading up to the 29 years of #Steady Progress from Mzee and his NRM party. This was certainly not the things he fought for in the bush-war or the idea he sold at the time. It has surely changed. I could have displayed all the corruption cases, embezzlements and such, but as I said this gist of the actions from the NRM party and Mzee. We have not taken in account his foreign adventures and military actions, or all the people that have been left behind. Peace.

Draft Estimate for the Budget for the Financial Year of 2015/2016 in Uganda – Quotes and Outtakes

Draft estimate for the Financial Year of 2015/2016 and how it’s expected to be. It will be a bunch of numbers and I have picked the ones that seem special. The ones that could be questioned and ask yourself why they use so much money on? That tells about how much the government of Uganda planning to use on certain pieces of civil service and ministries that they run. Everybody that wants to look and understand a bigger picture will get a bit more information. But even if this feels like a long piece. Remember the document that was swallowed into this was close to 1200 pages. So that I have written a long piece it’s a reason why and how it became this long. Hope your get some insights and it was worth my time.

Piece by Piece, Government Organization and Ministry:

Office of the President:

The Salaries are the same from 2014/2015 to 2015/2016. No change the same 26,233,125 UGX. The rest of the expenditure is the same except for the secret payment between the financial years and that is the “Classified Expenditure” which goes up sustainably from 2014/2015 when it was 11,069,633 to the next 2015/2016 it becomes 18,069,633. So it means that the Office of the President has one expense that goes to something secret and is up 7,000,000 from last year and the only one(Draft Estimates P: 30). There has even been another classified expenditure that is set for 3,940,034 UGX and this is not for the main Office of the President, but to the specific program of “Monitoring and Evaluation” (Draft Estimates P: 35). Total for the Office of the President is 53,835,847 UGX (Draft Estimate P: 42).

Statehouse:

Another “Classified Expenditure” is set for the FY 2015/2016: 36,700,000 UGX (Draft Estimates P: 44). The Total for the Statehouse is 253, 226,426 UGX (Draft Estimate P: 43).

Office of PM:

Total budget for the Office of the Prime Minister is set to be for the FY 2015/2015: 146,581,639 UGX (Draft Estimate P: 82).

UPF:

“Construction of Pakwach, Kabale, Morulem, Napak Police stations completed; Construction of a staff accommodation block of 4 units at Alebtong completed; Armouries constructed at Ikaffe, Kabalye and Olilim PTS; 10 vehicles procured for PRDP districts; ICT machinery and equipment (communication equipment) procured; Office furniture for Buliisa, Aleptong, Bukwo and Yumbe procured)” (Draft Estimates P: 25). The estimated budget for the UPF is 4bn for the FY 2015/2016.

IGP:

Gen. Kale Kayihura Salary is from next year 103,200,000 UGX (Draft Estimates P: 24).

Uganda Police Force:

Directorate of Counter Terrorism in FY 2015/2016 is 10.254.176 UGX (Draft Estimate P: 834). Construction of RD Agency HQs budgeted to 10.000.000 UGX is FY 2015/2016. Financial from External support for some of the Projects of UNRA:

Design for the New Nile Bridge at Jinja is supported from Japan. Vision of the Ministry:

Internal Security Environment:

“The tensions which erupted in the Rwenzori Sub-region in July 2014 were contained by the Security forces. This has greatly improved payment of salaries as they were received before the 28th day of every month. During this FY 2014/15, the ministry plans to retire 1,112 personnel in phases” (…) “National Medical Stores (NMS), the ministry further strengthened the implementation of this directive by entering into a Memorandum of Understanding with National Medical Stores. Afterthought:

The opinions and wishes from the Statement are clear. You can see the wishes of the UPDF. The way the government wishes to use and function of the army. From being an important part of the African Union peace missions. To also becoming vital in the close area like in South Sudan where even the advice from the Parliament is to pull out. We all know that isn’t going to happen because of the relationship between Museveni and Kiir. They have fought together and helped each other. Now Museveni supports his neighbor and the SPLM. The UPDF are also already in Somalia and Central African Republic as well. When you see the budget for things you well ask yourself. As well as the Classified Equipment are big number and you starting to wonder what that money goes for. Especially when they call it ‘Classified’ and why it’s that size of cash. But I think the rest of the numbers talk for them self. Peace.

Uganda – Bill No. 13 – The Appropiation Bill 2015 (29.05.2015) – Use of the Consolidated Fund- Quotes and Outtakes

This blog will be about the Bill No. 13 – ‘the Appropiation bill, 2015’ of the 29th May 2015. That was a part of the Uganda Gazette No. 28 CVIII dated 29th May 2015. This bill will the money used for service until 30th June of 2016. This bill is an extension to the Public Finance Management Act of 2015 and the Section of 17 in that Act. That states this:

Here is how it’s been scheduled to spend the fund in the different areas from the fund and use the Consolidate fund and using the fund accordingly to the Section 17 in the Public Finance Management Act of 2015.

I will not take every single expenditure that will be to many, but will take those that are striking. This is because the total Sixteen trillion, one hundred eighty one billion, six hundred twelve million, and six hundred thousand shillings for this Financial Year (FY), I will take those who strikes my mind and hopefully you find it interesting to. Here we go:
